Teacher Endorsement
An Endorsement Requirements Worksheet for the subject area to be taught should be submitted prior to the application for admission. Complete the top portion and columns 3 and 4 and deliver or mail with copies of transcripts to the address below. The Licensure Specialist will evaluate the worksheet and respond by mail.

The remaining requirements may be completed before or after admission to the program but must be completed prior to the internship. They may be completed by:
- Taking course work at any 2- or 4-year accredited institution
- Taking a graduate-level course to be used for the program elective with advisor approval
- Passing the College Level Examination Program (CLEP) tests for some endorsement courses. Registration information and a list of nearby testing sites is available in Robinson Hall A, Room 307 or at www.collegeboard.com
Provisionally licensed teachers may submit the state's evaluation of course work instead of the Endorsement Requirements Worksheet.
